Emergency treatment

If emergency medical treatment is considered necessary and the named contacts cannot be reached, I authorise Horangi Tang Soo Do to arrange appropriate emergency assistance for the student.

Membership communications

I agree to receive essential emails concerning membership, classes, licences, gradings, renewals and important association information.

Privacy notice

I confirm that I have read and accepted Horangi Tang Soo Do’s Privacy Notice and understand how the information supplied on this form will be used and stored.

Training risk

I understand that martial arts training involves physical activity and carries an inherent risk of injury. I agree that the student must follow instructors’ directions and report any injury, illness or difficulty.

Accurate information

I confirm that the information provided in this application is accurate and complete to the best of my knowledge. I will inform Horangi Tang Soo Do if relevant information changes.

Rules and code of conduct

I agree that the student will follow Horangi Tang Soo Do’s rules, safeguarding expectations and code of conduct.
